Online Learning Opt Out Form

Opt-out process

Parents/guardians may grant permission to have their child opt out of the mandatory online courses required for graduation. Students aged 18 or older or 16 or 17 who have withdrawn from parental control can also independently opt out of the graduation requirement. 
 
Please complete the form below. There are two options (choose 1):
  1. Online version (fill out online and submit)
  2. PDF version (print, fill out and hand in to the school's Student Services/Guidance department). Paper copies are also available from the school Student Services/Guidance departments.
